All services are subject to availability, and we reserve the right to modify or discontinue any service at our discretion.
3. Appointments
Scheduling: Appointments can be scheduled by calling our Muscatine or Tipton offices. We recommend booking in advance to ensure availability.
Cancellations: If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Failure to do so may result in a cancellation fee.
Late Arrivals: If you are late for your appointment, we will do our best to accommodate you, but your appointment may need to be rescheduled if it significantly affects our schedule.
4. Payment and Billing
Fees: Fees for services will be provided at the time of your appointment. Payment is expected at the time of service unless other arrangements have been made.
Insurance: We accept most major insurance plans. It is your responsibility to verify coverage with your insurance provider. Any co-pays, deductibles, or services not covered by insurance are the responsibility of the patient.
Billing: You will receive a bill for any outstanding balances. Payment is due upon receipt. We offer various payment options, including cash, credit/debit cards, and CareCredit.
